It looks like when you fill the form it just gives you a very small confirmation, I would believe that given a few weeks post submission, you will probably get something in the mail.
How long ago have you filled the form?

I filled out the order form after I saw that they had a Canada Maple Leaf option not thinking they would ship to Canada, the US site doesn't usually. I also purchased a bag and it came with the make it personal tag. I went online and submitted it? I think? Did you ever get this resolved?

Interesting, I recently purchased a new Staff Mid Size Bag and no such offer for patches came with that bag.......you can get customization (name, logo, etc.) when you order the bag thru the Titleist account you ordered the bag from when you ordered the bag........I have ordered a number of Titleist golf bags......the Linksmen Bag has an add on metal tag with your initials that fit on the bag that you can get from Titleist for free......
